Exalytics Encounters Error when Attempting to Boot from Release 2.0.0.0.0 Image - "No Driver Found" (Doc ID 2168250.1)

Symptoms

Attempting to upgrade an existing Exalytics X4-4 server to Release 2.0.0.0.0 by following the steps in the Installation Guide.  After booting the machine with the CD ROM and after passing a couple of screens, installation gets stuck and reports error:

"No Driver Found"
