SL150/HP LTO6 - The SL150 is Showing as Inoperative and All Drives in the Partition Became Unusable (Doc ID 2309669.1)

Information in this document applies to any platform.
The customer reports it appears that the offending drive caused the entire partition and all the drives in the partition to become unavailable and the partition itself was unusable. The partition had to be recreated.

Symptoms

1. Top drive in Module 4 has failed to unload the tape 101999L6 and move it to module 6.
2. Error "source drive requested is not configured".
3. Error "mailslot failed to pop open".
