Symptoms
Database instance fails to start on one node with following error. Database instance startup via sqlplus works fine.
[oracle@<EXADATA_DB_NODE_HOSTNAME>~]$ srvctl start instance -db <DB_NAME> -i <DB_NAME>2
PRCR-1013 : Failed to start resource ora.<DB_NAME>.db
PRCR-1064 : Failed to start resource ora.<DB_NAME>.db on node oraed02dbadm02
CRS-2674: Start of 'ora.<DB_NAME>.db' on 'oraed02dbadm02' failed
CRS-2678: 'ora.<DB_NAME>.db' on 'oraed02dbadm02' has experienced an unrecoverable failure
CRS-0267: Human intervention required to resume its availability.
CRS-5802: Unable to start the agent process
