Symptoms

Tape and drive LEDs are flashing.  Power cycled tape drive did not help, will not accept tapes.

Tapes will not load and have re-ipl'd the drive.

Error showing this in splat ASC/ASCQ: 5300h, Drive Error Code: 767Bh
