Exadata: Cell Service crash with RS-7445 [SERV CELLSRV HANG DETECTED] during a flash disk failure (Doc ID 2486713.1)

Symptoms

Customers may see Cell Service crash on Storage cells during the flash disk failure. The issues will start with lot of "NO IO COMPLETION ON DISK" messages written for the failing flash disk and Cell service eventually restart. Since the flash disk failed, the subsequent Cell Service restart would detect the failed flash disk as missing. This can result in the drop of the grid disks mapped to the failed flashdisk, in a write-back configuration.
