Symptoms

An I/O failure is reported without an attempt to complete the I/O on all of the available paths to a LUN.

Changes

This problem may occur whenever more than four paths exist to a LUN attached over Fibre Channel, or more than six paths for any other transport type.

For example, an LDOMs guest domain using Fibre Channel HBA SR-IOV virtual functions reported an I/O failure when an I/O domain associated with the same physical HBA was rebooted.

Three retry events were logged to the FMA infolog.
