Symptoms

A port went offline on an Oracle branded QLogic Dual Port 16Gb Fibre Channel QLE8362 HBA in a direct connect point-to-point unsupported configuration with a NetApp storage array. The port remained offline due to being forced into diagnostic port mode, even though the Diagnostic Port (D_Port) feature was disabled. The link status was "Diag Port" in output from the QLogic QConvergeConsole CLI (qaucli).
