[ PCA 3.x ] Termination of an Instance Image Can Lead To Premature Deletion of Boot Volumes Created From It

Symptoms
Communications with several running Compute Instances were lost, seemingly after a reboot of a management node.
Upon attempt to restart the instance it was discovered that the boot volume had been deleted from the ZFSSA system.
Changes
An issue with Vault led to problems with name lookup, which exposed a flaw in the code determining whether the "orphaned" boot volumes should be deleted.
The same issue caused problems for pods in the management nodes, the solution for which required a "rolling reboot" of them to resolve.
